My name is Tammy Schlewitz RN, BSN and I am the Elk Mound School District Nurse. My goal is to promote safety in the schools and community and promote academics by reducing barriers to learning. Through various health promotion measures, I strive to support physical, psychological, and emotional health and keep students in the classroom as much as possible.
On this website, you will find information on our health and screening programs, immunizations and requirements, medication policies and procedures, school illness and injury procedures, and information on when a child should stay home from school due to illness or injury. You will also find various health related documents/forms and information on head lice.
Parents/guardians are requested to inform the school of any health problem and/or special health care procedures that need to be performed at school. This will assist the school in developing a plan to best help the student meet his/her needs.
If your child has any of the following, please notify the school nurse to address their needs, implement health plans, or address additional concerns: (Health plans need to filled out annually)
- Asthma
- Diabetes
- Heart Condition
- Seizures
- Severe Allergies
- Or any other condition that may require an emergency care plan or procedure during school time
- Vision or hearing concerns
HEALTH AND SCREENING PROGRAMS
Physical Examinations
Kindergarten students are strongly encouraged to have Physical, Dental, and Vision examinations prior to the start of the school year.
Screenings
Students that are in PreKindergarten, Kindergarten, 1st, 2nd, 3rd, and all new students will have hearing screens performed at the school. Students that are in PreKindergarten, Kindergarten, 1st, 2nd, 5th, 8th, and all new students will be vision screened. Please call your school if you would like volunteer to help with these screenings. Screenings are also done at the request of a teacher or parent, etc. for all students throughout the school year. Waiver forms are available for any screenings a parent/guardian would like to decline.
Immunizations
This law requires students through grade 12 to be immunized according to their age/grade by the beginning of the school year.
- Parents are required to have their children immunized on schedule and keep accurate records. If a student is unable to receive immunizations for medical, religious, or personal conviction reasons, a waiver must be signed and given to the school
- Schools are required to keep records and review annually by the Immunization Law
- When students do not meet the requirements of the law:
- Parents are notified by legal notice.
- If the student is not in compliance after receiving the legal notice, the district attorney is notified.
Helpful Hints:
*Keep a copy of your child's immunization record. Notify your child's school each time your child receives an immunization.
*Free immunizations are available through the Dunn County Public Health Department for people that meet eligibility requirements. The health department is located in the basement of the Dunn County Government Center.
*There is a clinic the second and fourth Thursday of each month from 3:30 to 6:00 pm. A parent must accompany anyone under the age of 18 years. Call the Dunn County Public Health Department at (715) 232-2388 to make an appointment or get more information.

Medication Policies and Procedures
MEDICATIONS:
Please give medications at home whenever possible. However, if medication is needed to be dispensed during the school day, the following is necessary:
Prescription Medications
(1) A doctor's signature with written instructions needs to be on file.
(2) Written consent from the parent for the school to give the medication needs to be on file.
(3) Medication needs to be in a properly labeled prescription bottle.
Non-Prescription Medication
All non-prescription medications including acetaminophen, ibuprofen, anti-histamines, and cough drops must be supplied by the parent/guardian.
(1) Written consent from the parent with instructions for the school to give the medication needs to be on file.
(2) Medication must be in the original container.
- Students may not carry medication at school with the exception of prescription inhalers and epi-pens. These must be kept in student’s backpacks and medication forms still need to be filled out
- A parent/guardian needs to deliver medication to the nurse’s office to ensure proper forms are filled out
- A new medication form is needed for each school year

School Illness and Injury Procedures
Students who become ill or injured at school will be seen in the nurse’s office or will be assisted by school staff where the injury occurred or where student became ill. If your child is unable to return to class, a parent/guardian will be contacted. For your child's comfort, please make arrangements to pick them up as soon as possible.
When Should Your Child Stay Home Because of Illness or Injury?
FEVER: Students need to stay home when his/her temperature is greater than or equal to 100 degrees. Students must be fever-free, without the use of medications to lower fever, for 24 hours before returning to school.
VOMITING: Students need to stay home when they are vomiting. They need to be free from vomiting for 24 hours before returning to school.
DIARRHEA: Students need to stay home when they have diarrhea and be free from having episodes for 24 hours.
RASH: Students need to stay home when they have a rash that is open and draining or a rash with a fever.
CONTAGIOUS DISEASES: Students need to stay home when they have strep throat, impetigo, purulent pink eye. Students may return after 24 hours of treatment with an antibiotic.
NUISANCE DISEASES: Students with head lice, scabies, or ringworm must be treated before returning to school and cleared by the school nurse.
CHICKEN POX: Students with chickenpox need to stay home and are able to return to school 5-7 days after onset of rash, or when all sores dry and scabbed over.
- Please notify your child's school if your child is absent due to illness or injury.
- If your child has a serious illness or injury that affects school participation, please notify your child’s school, so a plan can be implemented to meet your child's needs.
